Product Manager

Primary Responsibilities

The Product Manager will:

• Own and evolve the product roadmap across clinical and business applications, balancing strategic bets with operational demand using a structured prioritization framework.
• Lead product discovery by engaging clinical, business stakeholders, and end users to identify workflow pain points and translate findings into clear problem statements.
• Write product requirements, user stories, and acceptance criteria, collaborating with technical and vendor teams (e.g., Epic/OCHIN, Salesforce, TalkDesk) to scope, sequence, and de-risk delivery.
• Maintain the product backlog, balancing new capabilities, enhancements, and technical debt.
• Define success metrics and KPIs for each initiative, track adoption and outcomes post-launch, and use data to inform future roadmap iterations.
• Partner with leaders in PACE Center Operations, Clinical, Finance, Strategy, Growth, and Health Plan Operations to align product direction with business strategy.
• Drive adoption of new rollouts by addressing training gaps through documentation, demos, and change management.
• Coordinate with multiple vendors, escalating to leadership when vendor accountability for scope, documentation, and delivery timelines is required.
• Facilitate stakeholder and steering meetings, representing the product’s voice to senior leadership.
• Operate with high autonomy, making independent judgments to resolve issues and drive outcomes.

Qualifications

The ideal candidate:

• Aligns with Habitat Health’s mission and values and is passionate about delivering care.
• Has 3+ years of experience as a Product Manager or Product Owner, preferably in healthcare, health tech, or highly regulated operational environments.
• Demonstrates expertise in running discovery, writing clear requirements/user stories, and prioritizing backlogs using structured frameworks.
• Uses success metrics and data to make and defend prioritization decisions.
• Possesses excellent communication skills and the ability to facilitate cross-departmental meetings with senior leadership and line-level staff.
• Has strong documentation skills, particularly for product requirement docs, roadmap artifacts, and board-ready materials.
• Is proficient in MS Office suite, backlog/roadmap tooling, and visualization tools.

Nice to Have

Experience with:

• Salesforce, TalkDesk, and/or Epic systems.
• Formal product management training or certification (e.g., CSPO, Pragmatic Institute).
• Agile methodology, including working with SCRUM teams.
• Healthcare interoperability standards (e.g., FHIR) or EHR data models.
• PACE program experience.
