We’re hiring an Ambulatory Clinical Pharmacy Specialist who will play a hands-on role in elevating patient care at a nationally recognized Academic Medical Center. In this dynamic, patient‑centered position, you’ll build meaningful relationships with patients and providers while delivering high‑quality medication management. You’ll work remotely, collaborating directly with care teams and practicing under a Collaborative Practice Agreement (CPA) to conduct virtual patient assessments, order and monitor evidence-based treatment plans, optimize therapy, and resolve medication-related problems.
Location: Remote role supporting the Centralia/Olympia/Lacey area
- Potential Locations: Providence Centralia Hospital, Providence St. Peter Hospital
What You’ll Do:
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards.
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
- Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.
- Support quality‑improvement and REMS initiatives.
- Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.
What You Need:
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in Washington
- Candidate must meet one of the following qualifications:
- Completion of a PGY2 Ambulatory Care Residency
- Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
- At least 3 years of ambulatory practice with disease state management
- Strong communication, organization, and tech skills
- Empathy, integrity, and a team-first mindset
- Solid computer skills are required, including proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint) for communication, documentation, and data management, as well as navigating the internet and web-based platforms.
