At Twin Health, we empower people to improve and prevent chronic metabolic diseases, like type 2 diabetes and obesity, with a new standard of care. Twin Health is the only company applying AI Digital Twin technology exclusively toward metabolic health.

We start by building a dynamic model of each person’s metabolism β€” drawing on thousands of data points from CGMs, smartwatches, and meal logs β€” that maps their personal path to better health. Guided by a dedicated clinical care team, our members have lowered their A1C below the diabetes range, achieved lasting weight loss, and reduced or even eliminated medications, all while living healthier, happier lives.

Responsibilities

As a Clinical Delivery Coordinator, you will play an essential role in optimizing Twin Health's clinical operations, ensuring every member has the right care team assigned in accordance with licensure and operational guidance. This role combines data analysis with early-stage responsibilities in Clinical Delivery Management (CDM) operations β€” including care team assignments, load balancing, supporting assignment-workflow automation, and supporting future-state integrations and data migration across platforms (e.g., Salesforce, Medallion, Clinicapp, Zendesk).

You'll collaborate with cross-functional teams across CDM work streams β€” Eligibility Ops, Licensing Ops, Member Experience, Clinical Ops, and Launch Readiness β€” focused on data-driven decisions, improved member care, and streamlined clinical processes.

Operational Support

  • Support daily provider assignments (based on licensure) and coach assignments (based on operational requirements) and guidance provided by practice management based on hiring models
  • Monitor care team panels to ensure they meet operational requirements based on program type, time in program, and operational requirements and changes (new hire onboarding, attrition, PTO coverage, licensure relapses, Language preferences like Spanish, etc.)
  • Provide prompt, professional responses to providers and coaches regarding care team and assignment requests
  • Coordinate with internal stakeholders to resolve assignment/reassignment queries in a timely manner
  • Perform data validation and quality checks to ensure assignment accuracy
  • Update and maintain assignment trackers and reports. Make data-driven decisions to inform product requirements based on operational needs.
  • Collaborate with the Clinical delivery, Practice management, Licensure and clinical team to resolve assignment discrepancies
  • Create and follow SOPs and escalate issues when necessary

Process Improvement & Quality Assurance

  • Identify operational inefficiencies through data analysis and recommend process improvements to management
  • Use AI tools such as Claude to improve productivity and streamline workflows
  • Maintain data accuracy while meeting turnaround-time expectations
  • Support QA initiatives for cross-functional processes, focused on cycle-time reduction and effort streamlining
  • Ensure analytics and reporting adhere to applicable SOPs and guidelines

Stakeholder Collaboration & Communication

  • Serve as a liaison between Cross functional team (CDM, PM, Clin ops) and technical/product teams on operational requirements related to assignments, reassignments, automations
  • Collaborate with Salesforce and product/partner integration teams to fulfill assignment-related project needs
  • Provide regular operational updates on assignment status to leadership and cross-functional partners
  • Maintain strong working relationships with internal and external stakeholders

Reporting & Analytics

  • Generate and maintain regular reports on care team assignment status, panel balance, and coverage gaps
  • Track key metrics such as time-to-assignment, panel load distribution, and reassignment volume
  • Build and update dashboards/trackers to give leadership visibility into assignment operations
  • Flag trends or anomalies in assignment data (e.g., unbalanced panels, licensure mismatches, offboarded provider assignments) for team review
  • Support ad hoc data pulls and analysis requests from CDM leadership and cross-functional partners
  • Ensure reporting is accurate, timely, and consistent with data definitions used across Salesforce, Medallion, and other source systems
  • Other duties as assigned

Qualifications

  • Proficiency in Microsoft Excel/Google sheets (formulas, filters, sorting, pivot tables, lookup functions, and data analysis).
  • Familiarity with Claude or other AI productivity tools.
