Core Responsibilities
As a Care Navigator, you will build trusting relationships with patients using motivational communication techniques to drive positive health behavior change and improve patient outcomes. This role focuses on triaging vital signs and leveraging this data to promote lifestyle and health behavior changes through collaboration with the patient’s care team.
The ideal candidate will conduct wellness calls with patients to outline patient-centric goals and develop action plans for better health and well-being. You will monitor physiological markers such as blood pressure, weight, and blood glucose via a Remote Care Platform that receives electronically transmitted data.
Key responsibilities include:
- Manage physiological markers with clinical appropriateness and meet team goals outlined by metrics.
- Significantly impact longitudinal patient engagement in RPM programs.
- Provide preventive health and disease management education and coaching.
- Perform monthly wellness calls with assigned patients.
- Lead collaborative wellness calls to define health goals with the patient’s Care Team.
- Manage patient messaging and alerts.
- Direct patients to treating physicians for routine questions.
- Follow escalation pathways for urgent care needs.
Key Qualifications
The ideal candidate must have:
- Unrestricted RN/LPN/LVN license in a compact state and Michigan.
- Ability to work EST zone between 8:00 AM - 5:00 PM.
- Spanish-speaking skills are a plus.
- 1-3 years of clinical experience, patient management, or disease management.
- Experience working with different provider practices and workflows.
- Experience with Medicare patients and performing virtual visits/telephonic care management.
- Fast learner and ability to work independently with minimal direction.
- Motivational Interviewing/Health Behavior Change experience is a plus.
- Health Coach certification is a plus.
