The Care Coordinator is a patient-focused role responsible for navigating the comprehensive care of our patients throughout their healthcare journey. This position ensures a delightful experience through empathy, clear communication, and simplicity. The role prioritizes and documents administrative tasks in our internal tools, manages inbound/outbound calls, and collaborates closely with our clinical team to resolve patient issues. This position reports to the Operations Manager and works with the Director of Operations, Medical Director, and clinicians.
Job Essentials:
- Appointment scheduling, notification, reminders, and troubleshooting
- Handling inbound calls and placing outbound calls to patients, provider offices, insurance companies, labs, and other stakeholders involved in patient care
- Preparing, organizing, submitting, and managing care documentation in electronic medical records systems and other internal tools
- Supporting the clinician team with a proactive mindset, including prompt responses and high attention to detail
- Supporting patients' health with empathy, clear communication, seamless transitions, and simplifying processes
- Demonstrating knowledge of HIPAA regulations and maintaining the confidentiality of patient information to comply with internal policies
- Collaborating with the team to deliver a delightful patient experience
- Being self-motivated, organized, and possessing excellent follow-up and follow-through skills
- Other duties as assigned
Minimum Qualifications:
- A high school diploma or equivalent
- One year of experience in a healthcare-based patient service role requiring the use of multiple software systems
- Proficiency in computer software, including electronic medical records, word processing, spreadsheets, and calendaring
- Excellent problem-solving skills and the ability to navigate ambiguity
- Effective verbal and written communication skills, with the ability to thrive in a virtual environment
- Knowledge of medical record management, medical terminology, and healthcare billing practices
- Time management and prioritization skills to manage multiple tasks successfully
Preferred Qualifications:
- Experience with using the Athena platform
- Experience with medical billing submission, clarification, and troubleshooting to optimize reimbursement
- Experience collaborating with engineering and product teams
- Experience with specialist referral processes, including insurance authorization and documentation requirements
- Multi-lingual proficiency
