Being a Software Engineer at AIOS

As a Software Engineer at AIOS, your fundamental role is to build exceptional apps, tools, and services for our customers, clinicians, and internal teams.

Our systems serve >200k patients each month. Our agentic workflows autonomously generate >$100k of revenue per day. Our customer products support complete healthcare journeys, while our clinical systems help clinicians deliver care safely at enormous scale.

You’ll join one of these teams:

  • AI Growth: Build self-improving growth loops across visitors, leads, and prospective customers. You’ll work across customer-facing AI, internal automation, experimentation, and growth products.

  • Product Growth & Retention: Make checking in, reordering, and continuing treatment effortless. You’ll build in React Native and help make our mobile app the core product customers rely on throughout treatment.

  • Engagement: Build the best healthcare app in the UK for patients and non-patients alike. You’ll create useful, engaging products that give people a reason to open the app regularly and build a lasting relationship with our brand.

  • Care & Aegis: Build the clinical workflows, tools, data flows, and backend systems surrounding Aegis, our AI-assisted clinical decision-support system.

During the interview process, we’ll understand where you do your best work and find the team where your strengths can create the most leverage.

You’ll share ownership of the product itself: what gets built, how it works, how it feels to use, and whether it produces the outcome we expected. You’ll understand the people using your work, collaborate closely with product and design, and question decisions that do not make sense.

Your ownership continues in production. You’ll follow how people use what you built, understand the relevant metrics, identify where the experience falls short, and keep improving it.

We care deeply about the quality of our software. The systems you build must be clear, tested, observable, and maintainable by the engineers who come after you. You’ll make thoughtful architectural decisions, handle failure cases properly, review code seriously, and strengthen weak foundations before they become permanent constraints.

Speed matters just as much. At our pace, excellent engineering means finding the simplest sound design and shipping it quickly. Move fast, build well, and leave foundations that can carry what comes next.

We provide Claude Code and Codex subscriptions. We expect you to use them to increase your speed and ambition while retaining complete responsibility for your judgment, understanding, and work.

This role is FTE and remote, with some in-person working days with the Kosovo engineering team.

You’ll report directly to the VP of Engineering.

You’ll also work closely with:

  • Head of Product

  • CEO

Key responsibilities

  • Build Products End to End: You’ll work closely with product and design to understand the problem, shape the solution, build it across the stack, and carry it through production.

  • Own the Experience: You’ll care about the complete experience, including edge cases, loading states, performance, reliability, and the small details that determine whether a product feels excellent.

  • Build to Last: You’ll write clear, tested, observable, and maintainable software. You’ll choose simple architectures, handle failure cases properly, and strengthen the foundations beneath your work.

  • Own Production: You’ll instrument what you ship, follow the relevant metrics, investigate failures, and keep iterating until the product creates the intended outcome.

  • Use AI as a Force Multiplier: You’ll use Claude Code, Codex, and other AI tools throughout development to move faster, explore more ambitious solutions, and increase the quality of your work.

  • Raise the Engineering Bar: You’ll review code seriously, communicate decisions clearly, share context, improve weak systems, and help the engineers around you do better work.

Need to have

  • Experience: You have 3+ years of professional software engineering experience and have shipped meaningful products used by real customers.

  • Full-Stack Engineering: You can deliver across frontend and backend systems. You may be significantly stronger on one side, but you are comfortable working across both.

  • Product Mindset: You care about what should be built, how it feels to use, and whether it creates the intended outcome.

  • Engineering Quality: You write clear, tested, observable, and maintainable software. You understand architecture, databases, APIs, failure modes, and production reliability.

  • Ownership: You acquire context proactively, raise uncertainty early, follow your work into production, and take responsibility for reaching the outcome.

  • Judgment: You can balance speed, simplicity, and long-term maintainability. You push back clearly when a technical or product decision does not make sense.

  • AI-Native Execution: You use AI development tools effectively and remain fully accountable for the quality and correctness of the work they help you produce.

Nice to have

  • TypeScript: You have strong experience building production systems with TypeScript.

  • Our Stack: You’ve worked with technologies such as React, React Native, Expo, Next.js, NestJS, GraphQL, Prisma, PostgreSQL, Redis, queues, and AWS.

    • Note: Strong React Native experience is required for the Product Growth & Retention and Engagement openings.

  • Applied AI: You’ve built customer-facing products, internal tools, or automated workflows on top of LLMs.

  • Experimentation: You’ve instrumented products, run experiments, and used production data to decide what to improve next.

  • Product Taste: You care about UX details and work effectively with designers to produce thoughtful, polished experiences.

  • Startup Experience: You’ve worked in an environment where speed, ambiguity, and individual ownership were real.

  • Education: A bachelor’s degree in Computer Science or a closely related technical field is a strong advantage.

Our cultural standards

We aim to make this your life's work. This should be the most challenging, most rewarding role of your life. Accordingly, these are the core cultural standards to which we hold ourselves & our team-members:

  1. Belief in the mission: We will have served 100 million patients by the end of 2035 and we transform the life of most patients who join. We have a lot of work to do. We are obsessed with our patients and are dedicated to the mission.

  2. Unwavering integrity: We are at the frontier, so we often live in ambiguity with no trodden path. When we can't look to others for guidance, we must maintain impeccable ethics and unwavering integrity.

  3. Only the paranoid survive: Bad sh*t is coming. By joining us, you're choosing to sail straight towards the storms with unhesitating conviction. However much we've already done, however far we've already come — it's still Day 1 and all our work is ahead of us.

  4. If we're average we fail: We are only interested in "insanely great", a focus on the quality of our execution that in everyday life would be considered pathological. We have a dedication to excellence and reject incompetence.

  5. Commitment to candor: That which can be destroyed by the truth should be. You get full transparency from the company and the company expects full transparency from you. We never say anything about someone that we wouldn't say to them directly. We give feedback with love and do not need to protect people from fleeting physical sensations.

  6. A maniacal sense of urgency: We execute at an intensity that most people think is impossible. Speed is critical and we need things done yesterday. We all work very hard and in such a competitive world there really is no other way to win.

  7. Enduring frugality: We are frugal. We hate being wasteful and we are anti-luxury. A culture of cheapness keeps us young. We spend our cash wisely & carefully — in a way that would make our grandmas proud.

  8. Bulldozing barriers: The world is malleable and we shape it. We truly believe this and act accordingly. We are relentlessly resourceful and are at the mercy of no-one but ourselves. You'll be shocked how capable you are and how much you can achieve.

  9. Keep your head down: We're boring people doing exciting work. We don't chase short-term status — we ignore short-term dopamine hits and focus on what matters. Outsiders will underestimate us and we revel in that.

  10. The power of focus: We live in a world of power laws and we cannot overestimate the unimportance of practically everything. Know your One Thing, and nail it.

Compensation & Benefits

You get paid above market salary and you get early stage equity, so you get really rich if we nail this.

  • Compensation: €40k-€80k, + equity

Benefits

  • Healthcare: comprehensive medical insurance

  • Vacation: PTO with a yearly minimum (≄2wks/yr + local national holidays)

  • Remote: our team is fully distributed across the world and functions fully remotely

  • Personal development: budget for books, courses, coaching ($1200/yr)

  • Personal wellness: budget for gym, health apps ($1200/yr)

  • Coaching: free biweekly health coaching

  • Equipment: Macbook & work-from-home equipment provided as needed

  • What are we missing? We're still early so you get to shape our culture.

Fella Health (operating under fellahealth.com, legally AIOS Inc.) is an enterprise-grade telehealth clinic, specialized men's metabolic health engine, and medical weight management platform engineered to dismantle the severe social stigma, clinical fragmentation, and lifestyle friction preventing busy men from accessing sustainable obesity treatments. Founded in 2021 by Cambridge University alumni Richie Cartwright and Luke Harries under the institutional incubation of Y Combinator, the corporation revolutionized the consumer digital health sector by building a personalized, science-backed care delivery network tailored specifically for men. Moving past historical weight-loss paradigms—which relied on inefficient, low-compliance "willpower-only" routines that fail long-term for roughly 90 percent of patients—Fella Health natively unifies board-certified clinical obesity evaluations, continuous medical oversight, direct-to-home deliveries of advanced GLP-1 and GIP metabolic medications (such as semaglutide and tirzepatide), and continuous 1:1 behavioral health coaching. Underpinning its hyper-scale distribution architecture is ClinicOS, the company's proprietary, AI-driven clinical operating system designed to automate administrative workflows, optimize patient-clinician triage, and enable consumer brands to launch direct-to-patient healthcare channels seamlessly at scale. Backed by marquee institutional venture capital firms including Y Combinator, BrandProject, and Global Founders Capital, alongside prominent angel backers behind tech giants like Indeed and Alan, the cash-flow positive enterprise has scaled its multi-brand footprint across the United States and the United Kingdom. Serving over 50,000 active members and generating upwards of $100 million in annualized revenue, the organization continues to pioneer scalable consumer biotech access. Headquartered in Austin, Texas, with additional offices in San Francisco, California, the company operates via a highly distributed, remote-first global workspace. What distinguishes Fella Health is its strict, no-nonsense integration of clinical precision and psychological re-conditioning; by connecting high-availability programmatic telemedicine infrastructure with advanced endocrine and metabolic pharmacology, the corporation remains a definitive vanguard in the global fight against chronic metabolic disease and modern longevity optimization.
