About the Role

Wider Circle is looking for a Care Advocate Manager, Care Delivery to join our circle and lead a remote-first team supporting high-risk members through our Community Health Integration (CHI) and Principal Illness Navigation (PIN) programs.

In this role, you will focus on team leadership, operational excellence, and cross-functional collaboration. You will guide a team of Care Advocates, helping them bridge the gap between clinical care and social needs for our members. The ideal candidate is a strong operator and a compassionate leader who relies on metrics to drive performance, optimize workflows, and collaborate effectively across the organization.

What You'll Do

  • Lead and Develop: You will manage and coach a team of at least 8 Care Advocates, driving performance, accountability, and engagement in a remote-first environment.
  • Collaborate Cross-Functionally: You will work closely with billing, product, operations, and clinical teams to streamline processes, solve day-to-day challenges, and ensure smooth program delivery.
  • Drive Metrics & Performance: You will take a highly data-driven approach to track key metrics, monitor team productivity, and ensure the achievement of program and documentation goals.
  • Optimize Workflows: You will leverage data and analytics to identify trends, eliminate bottlenecks, and drive continuous improvement across your team's operations.
  • Oversee Program Engagement: You will implement and measure strategies to engage and retain members in CHI and PIN programs, guiding your team to proactively re-engage those at risk of disengagement.
  • Ensure Compliance & Quality: You will audit your team's work to ensure all documentation meets CMS and program requirements for accuracy and completeness.
  • Act as an Escalation Point: You will serve as the first point of escalation for complex member cases, stepping in to coordinate with clinical and telehealth teams when your advocates need support.
  • Onboard and Train: You will onboard and train new team members on workflows, documentation standards, and compliance requirements.
  • Build Community Bridges: You will oversee the development and maintenance of partnerships with local community organizations to ensure your team can successfully connect members with necessary resources.

Requirements

  • Bachelor’s degree in healthcare, social work, public health, or related field, or equivalent experience
  • 5+ years of experience in healthcare, community health, social services, or healthcare operations
  • 1–2+ years of experience managing or leading a team (remote or field-based preferred)
  • Experience working with high-risk or vulnerable populations
  • Strong understanding of care coordination, documentation, and compliance requirements (CMS experience a plus)
  • Excellent communication, interpersonal, and organizational skills
  • Proven ability to manage multiple priorities while maintaining attention to detail
  • Comfort using digital tools, case management systems, and performance dashboards
  • Valid driver’s license and reliable transportation
