Registered Nurse - FuzeRx

About the Role: At Fuze Health, we put patients first and address the most pressing needs in healthcare. We are seeking a Registered Nurse to support the FuzeRx Central Operations' telehealth care team by providing remote outreach to Medicare members. This role focuses on ensuring member compliance with a new Stars measure and delivering compassionate, high-quality virtual care.

What You'll Do

• Provide remote patient care and support through telehealth platforms, conducting virtual patient assessments.
• Accurately assess and document patient conditions, histories, and care interactions in EMR systems.
• Educate patients and their families on medical conditions, medication adherence, and self-care practices.
• Work closely with other healthcare providers to develop and implement individualized care plans.
• Maintain accurate records, ensuring adherence to healthcare regulations and privacy laws.

What You Bring

• Bachelor of Science in Nursing (BSN) degree and an active, non-restricted compact state nursing license in an approved state.
• 2+ years of experience as a Registered Nurse, with 1–2 years in a remote healthcare or telehealth setting.
• Strong computer skills and familiarity with electronic medical record (EMR) systems.
• Excellent communication, customer service, and interpersonal skills, especially in high-volume or high-stress scenarios.
