The Role:
Are you passionate about helping patients navigate their medications and improve their health, and do you speak Mandarin or Cantonese? Join our Clinical Care team at Arine! As a Bilingual RN, you will play a vital role in supporting Chinese-speaking patients, enhancing care coordination, and helping them better understand and manage their medications - especially for conditions like heart disease and diabetes.
Using Arine’s innovative platform, you’ll conduct outreach to patients, providers, and pharmacies to perform medication reconciliation, provide education, and ensure adherence to treatment plans. You’ll collect clinical data, such as blood pressure readings, and work closely with pharmacists and other healthcare professionals to identify and resolve medication-related issues that may impact patient outcomes or lead to readmissions.
In this role, you’ll also support quality improvement efforts tied to HEDIS goals, help gather necessary documentation for CMS measures, and contribute to improving operational workflows. Your input will be instrumental in triaging high-risk patients and ensuring continuity of care across multiple settings.

What You’ll Be Doing:
- Conducting follow-up calls with patients post-discharge, in Mandarin and/or Cantonese and English, to facilitate medication reconciliation and ensure understanding of their medication regimens
- Providing patient education and adherence support on medications related to heart health and diabetes
- Collecting blood pressures from members’ and providers’ offices and offering support/education in helping achieve HEDIS goals
- Collaborating with pharmacists and healthcare providers to identify and address medication-related issues and health concerns that may lead to readmissions
- Monitoring patient care across the care team, ensuring continuity and adherence to treatment plans
- Documenting patient interactions and medication reconciliations accurately in Arine’s software platform
- Engaging with hospitals, clinics, and pharmacies to coordinate care and resolve clinical issues promptly
- Educating patients on their medications, potential side effects, and the importance of adherence to their treatment plans - in the language they’re most comfortable with
- Triaging high-risk members to a clinical pharmacist for in-depth medication reviews, side effect concerns, cost concerns, or when pill burden is a concern
- Participating in process improvement initiatives to enhance care coordination and patient outcomes
- Assisting with obtaining necessary referrals and authorizations as part of the care management process
- Working with providers’ offices to confirm medication histories and collect records for various CMS measures as needed
- Managing a consistent volume of outbound and inbound calls while maintaining productivity and service quality
- Meeting individual and team performance metrics related to outreach, engagement, documentation quality, and service outcomes
- Scheduling patients for telephonic or videoconference appointments with clinical pharmacists
- Preparing and sending patient educational materials and other necessary items as needed
- Following established workflows while remaining flexible as processes evolve
- Taking ownership of assigned work and seeing it through to completion
Who You Are and What You Bring:
- A Bachelor’s degree in Nursing (BSN) and/or an active Registered Nurse (RN) license in New York
- Bilingual proficiency in Mandarin and/or Cantonese and English is required - you’ll use these skills daily to communicate with and advocate for patients
- Prior experience in case management and a solid understanding of care coordination processes
- Strong knowledge of medication management, including medication reconciliation and related terminology
- Excellent written and oral communication skills in both English and Chinese, with the ability to effectively educate and support patients
- Strong computer skills, including proficiency in Google Workspace and Mac (Apple) environment, with comfort using collaboration and contact-center tools such as Slack and Five9, navigating multiple browser-based systems simultaneously, and performing basic technical troubleshooting
- Ability to prioritize effectively and manage multiple tasks in a fast-paced healthcare environment
- Demonstrated reliability, accountability, and follow-through
- High attention to detail with a strong commitment to documentation accuracy
- Comfort working in a metrics-driven environment where outcomes and quality are tracked

Nice-to-Haves:
- Experience with electronic medical records (EMR) systems
- Previous work in an ambulatory care setting, including taking medication histories and managing transitions of care
- Familiarity with pharmacy operations and medication therapies
- Certified Diabetes Educator (CDE) certification
Remote Work Requirements:
- An established private work area that ensures information privacy
- A stable high-speed internet connection for telephonic and/or remote work
- This role is remote, but you will be required to come to on-site meetings multiple times per year. This may be in the interview process, onboarding, and team meetings

This position is temp-to-hire for an expected duration of 6 months, with the possibility of extension or conversion to a permanent role based on performance and business needs. Throughout the temporary period, you will receive full benefits starting from day one.
The posted range represents the expected hourly rate for this position and does not include any other potential components of the compensation package, benefits, and perks. Ultimately, the final pay decision will consider factors such as your experience, job level, location, and other relevant job-related criteria. The hourly pay rate for this position is: $39-60 hour.
Seasonality & Schedule:
Our Clinical Care team operates within a seasonal business cycle, and we want candidates to have a clear picture of what to expect. During peak season (May through early December), our team plays a vital role in supporting business operations. Workload and scheduling demands increase during this time, and time off may be limited. Optional overtime may also be offered to meet business needs. Each year, the Clinical Care department observes a holiday shutdown of approximately 2–3 weeks between late December and early January. Please note that while Arine remains operational, the Clinical Care department is closed during this period. Following the shutdown, the team transitions back to full capacity on a modified schedule through February, during which some shifts or hours may differ from standard operations. Information Security Roles and Responsibilities:
